2011 Registration
Registration for the
2011 Lacrosse Season is closed. If you missed the deadline
and would still like to play, please e-mail club secretary
Kerri Smith for special
access to the on-line registration system.
Welcome to the 2011 Centennial Lacrosse registration site.
The official High School Season runs from February 15th through
the end of May. Registration fees are $225 plus $35 US
Lacrosse Membership fee. All players are required to participate
in team fundraising and there will be additional fees for
Varsity team travel to out of town games/tournaments.
There are four required steps to register for the 2011 lacrosse
season.

Establish/Renew US
Lacrosse Membership.
Note: you will need your membership number and expiration
date to register on the Centennial Site. Player’s
US Lacrosse membership must be valid through May 30, 2011.
- Complete Sports Physical/Health Form (download/print
below)
Physical/Health forms must be turned in to coach on or before
first practice. Players will not be allowed to practice
until this form is received.
